Enrollment Procedures
Students must be enrolled with the Sun Prairie Area School District and be attending Sun Prairie High School. Students can refer themselves or they may be referred by a parent, teacher, counselor or school administrator to Prairie Phoenix Academy. Students must fill out an application (available through their school counselor) and the CARE Team must approve the application. New students are accepted at Prairie Phoenix Academy at the beginning of each quarter. Once given approval the student must complete a transition plan with their counselor or the Prairie Phoenix Academy Principal. This transition plan will set goals in three main areas that must be met in order to make a successful transition from the high school to Prairie Phoenix Academy at the quarter. If goals are not met, the student will remain on the transition plan for another quarter.
