ATHLETIC REQUIREMENTS FOR STUDENTS PARTICIPATING IN CARDINAL HEIGHTS UPPER MIDDLE SCHOOL SPORTS

If your student is interested in participating in athletics at Cardinal Heights, they will need the following:
1.  The Participation & Physical Agreement Form 2011-2012             
2.  Code of Conduct:  Click on the link below for a copy of the Code of Conduct that parents and students need to sign.
3.  Athletic fee:  A one time $26 athletic fee which is good for all sports at Cardinal Heights.
