The following forms are available online for your convenience, and in the Athletic Department Office. 

Athletic Participation & Physical Form
Beginning with the fall of 2011 we will be using this new and updated form which will replace the green physical card, the tan alternate year card, and the rules and regulations sheet from the rules and regs booklet.

Parent/guardian and student sign this form to acknowledge permission to participate in interscholastic / school sponsored athletics as well as agreement to abide by the Sun Prairie High School Rules and Regulations.  This form is required for the first sport in which the student participates each year.

For each sport in which a student participates, a blue medical history card and athletic fee are also turned in to the athletic department office.

Travel Permission Forms
From time to time we realize there are circumstances that arise that make it necessary for a student to not take the provided school transportation to and from games.  Just a reminder that there is a travel permission form online as well as in our office, explaining the expectations that go along with this.  Student-athletes are expected to ride the transportation provided with their team.  Please do not ask a coach after a game to release your child to go with you if you have not followed the correct procedure with the athletic department office.  Remember that forms need to be filled out first by parents and then brought to the athletic department office to be signed at least one day in advance.  Thank you for your cooperation.
