ATHLETIC CODE MEETINGS:
Students along with their parents are expected to attend the high school code meetings at the beginning of each sports season.  Code meeting dates and times are listed on each sign-up information page.  These meetings are held in the Sun Prairie High School Auditorium (PAC).  Following the code meeting, squad meetings are conducted with coaches.

SPORTS PHYSICALS

Physicals dated before 4/1/11 have expired.
Physicals dated on or after 4/1/11 will be good for the 11-12 and 12-13 school years.  Physicals dated on or after 4/1/12 will be good for the 12-13 and 13-14 school years.

If you get a new physical during the year, remember to take a physical/participation form with you for your doctor to sign and stamp.  Remember to turn the new physical form in to the athletic department office to keep your records up to date.  The clinics do not automatically send us this information.

WHAT DO I NEED EACH SEASON?
For the first sport you do each year, you will need:
1.  Athletic Participation and Physical Form
2.  Blue Emergency Medical Card
3.  Athletic Fee

For each additional sport during the same school year, you will need:
1.  Blue Emergency Medical Card
2.  Athletic Fee

ATHLETIC FEES - REFUNDS:

Student-athletes that are cut from a team by a coach due to try outs, are eligible to have their athletic fee refunded.  Please stop in the athletic office within the first two weeks of the start of the sport season to pick up your fee.  The athletic department is not responsible for unclaimed fees, payments are not mailed if a student does not make a team.  NOTE:  Refunds are only good for the current school year.
