Please pick up all your child’s remaining medications on the last day of school unless they are going to summer school. Medication can NOT be sent home with the student. Medication that is not picked up will be disposed of at the end of the school year.

If you anticipate medication(s) will need to be administered at school next year, new medication consent forms will need to be completed.  For prescription medication, your health care provider will need to either complete and sign the “Medication Administration Consent Form” or provide a copy of their own electronic school medication form.

Please bring the labeled medication container and completed forms to the school office by the first day of the next school year.

All medications administered at school need to have the following information matching the doctor’s order (prescription) or parent/guardian order (non-prescription) printed on the original container:

1)Student’s full name

2)Name and dosage of the drug

3)Time to be given

4)Physician’s name

Thank you for your cooperation.  If you have any questions, please call me 834-7806.
