Athletic requirements for students participating in Prairie View sports: If your student is interested in participating in athletics at Prairie View, he/she will need the following:

1. Prairie View Participation Agreement 2011-2012: Click on the link below for a copy of the agreement that you will need signed and stamped by your doctor.
2. Code of Conduct: Click on the link below for a copy of the Code of Conduct that parents and students need to sign.
3. Athletic fee: A one time $26 athletic fee which is good for all sports at Prairie View.

Need a physical? Call Concentra in Madison, (608)244-1213, off Stoughton Road. They take walk-ins for sport physicals for $25.00.
