Committee Reimbursement Form

Please fill out the information below for expense reimbursement for ROSCO committee work.  It is important to be sure that all areas of the form are filled out.  The form will be submitted automatically to the ROSCO Treasurer and a copy will also be emailed to you provided you enter your correct email address at the bottom of the form.

All receipts for the expenses entered on this form will need to still be submitted to the ROSCO Treasurer for completion of the reimbursement.  The receipts should be mailed or left at school for Aleta Amick.  If mailing the receipts, the address for school is:
